Proof of Concept (PoC) Care Planner

Job Introduction

Our Healthcare segment is looking for 6 PoC Care Planners to join our dedicated team on a 5 weeks fixed term contract (14 November- 21 December 2022).

The role will run 0900-1700 Monday to Friday with the possibility of some on call support during weekend (0800-1600). Some flexibility with hours may be possible. 

The role holder will need to live in the Buckinghamshire area. This role will be predominantly home-based from weeks 2-5 as you'll be offering a telephony service to our outpatient clients. However, you are expected to travel to client’s home addresses once the clients have been identified (these will be within approx. 30 mile radius of Stoke Mandeville Hospital). The first week or two of the project you will spend much of your time attending meetings at Stoke Mandeville Hospital.

Role Responsibility

• Work as part of a small and experienced team to help demonstrate the viability of this new service offer through the first month scale up of a 4 month proof of concept (PoC).
• Support the Discharge Facilitators in ensuring timely discharge of hospital inpatients that are clinically fit for discharge and the meet agreed PoC criteria.
• Meet with clients enrolled on the PoC (and their carers where applicable) to develop and mobilise their home care plan as directed by the Discharge Facilitators.
• Put in place non-clinical support that will help reduce the chances of the discharged inpatients (clients) having future unplanned readmissions.
• Provide feedback to the Discharge Facilitator on this role, client and carer engagement, and any additional information that may help inform the future development of the service.
