Job Introduction

As an Onward Care Discharge Facilitator for Sodexo, based at Stoke Mandeville and Amersham Hospitals, you’ll play a vital role in helping patients transition from hospital to home with confidence, compassion, and the support they need to thrive.

This is not a clinical role — it’s a people-first, problem-solving position that supports patients who are medically fit for discharge by ensuring their home environment and local support services are ready for them. Working closely with NHS discharge teams, you’ll empower patients and carers, reduce the risk of readmission, and enhance recovery in the community.

What you’ll do: 

  • Liaise with NHS hospital teams to identify eligible patients and support discharge planning.

  • Meet patients and families to explain the service, gain consent, and create a tailored non-clinical care plan.

  • Mobilise community, voluntary, and tech-based support (e.g., home adaptations, wellbeing monitoring).

  • Conduct home visits and remote check-ins post-discharge to review wellbeing and support needs.

  • Provide non-clinical coaching on issues like nutrition, hydration, and daily routines.

  • Monitor wellbeing data and escalate concerns promptly, in line with agreed protocols.

  • Deliver accurate and timely documentation and ensure compliance with IG and QA standards.

  • Act as a trusted connector between hospital, community services, and clients.

What you bring: 

Essential:

  • Strong interpersonal and communication skills – you're empathetic, reassuring, and solution-focused.

  • Excellent organisational skills, with the ability to prioritise and respond quickly.

  • Based in Buckinghamshire with the ability to travel between hospitals and client homes.

  • Basic IT skills – e.g., comfortable using email or inputting notes

Desirable 

  • Knowledge of hospital discharge processes, local services, or social prescribing.

  • Experience in coaching, care navigation, or non-clinical case management.

  • Experience working in community, healthcare, or support settings.
