Clinical Pharmacist (Prescriber)

Job Introduction

Sodexo Justice provides healthcare services within HMP Forest Bank, HMP Bronzefield and HMP Peterborough.

We are seeking a Clinical Pharmacist to develop medicines optimisation at HMP Forest Bank. The pharmacist should be an Independent prescriber (we will consider training up the right candidate), who will take responsibility for areas of chronic disease management at HMP Forest Bank, undertake clinical medication reviews, manage patients with complex polypharmacy, as well as reviewing processes and improving patient experience.

They will provide the healthcare team with information with regards to prescriptions, medication queries and repeat prescription management. They will deal with acute prescription requests, medicines reconciliation on transfer of care and systems for safer prescribing and providing expertise in clinical medicines advice.

This role would be a great opportunity if you have previous prison, hospital or community pharmacy experience and are looking for a new and exciting challenge. The successful candidate will need to be self-motivated and able to work on their own, whilst also integrating into the wider healthcare team. They will have excellent organisational, interpersonal and communication skills together with a good working knowledge of the challenges facing medicines management in the NHS.

As well as the 16 residential units on site, there is a healthcare centre, gym, library, workshop complex, visits hall, chaplaincy and kitchen. The prison’s capacity stands at 1,460, holding adult men both on remand or sentenced and young prisoners between the ages of 18-21 years.

Role Responsibility

  • Act as responsible pharmacist when rostered to do so in the dispensary.
  • Undertake clinical screening and final accuracy checks when working in the dispensary.
  • Provide face to face clinics to help patients with questions, queries and concerns about their medicines.
  • Hold clinics for patients requiring face-to-face Clinical Medication Reviews (CMRs) i.e. a review of the ongoing need for each medicine, a review of monitoring needs and an opportunity to support patients with their concordance with prescribed medication.
  • Undertake clinical medication reviews with patients with multi-morbidity and polypharmacy and implement own prescribing changes (as an independent prescriber) and order relevant monitoring tests.
  • See patients in multi-morbidity clinics and in partnership with the multidisciplinary team implement improvements to the patient’s medicines, including deprescribing.
  • Run own long-term conditions clinics where responsible for prescribing as an independent prescriber for conditions where medicines have a large component (e.g. stable angina, warfarin monitoring and dose adjustment).
  • Managing caseload of patients with common/minor/self-limiting ailments whilst working within a scope of practice and limits of competence.
  • Prescribe and adjust prescriptions where required within competency and agreed areas of practice (if registered Independent Prescriber)
  • Answer relevant medicines-related enquiries from GP’s, other healthcare staff and patients with queries about medication.

The Ideal Candidate

Essential

  • Master’s Degree of Pharmacy or equivalent
  • Registered with the General Pharmaceutical Council of GB (GPhC)
  • Registered Independent Pharmacist Prescriber or working towards the qualification. We will consider training up the right candidate if they do not already have the qualification.
  • Previous prison/hospital/community pharmacy experience in the UK
  • Previous proven experience of providing clinical services in a healthcare setting
  • Experience of working in groups and providing prescribing advice.
  • Experience of writing guidelines, policies and standard operating procedures.
  • Experience of undertaking patient medication histories and medication reviews.

Desirable:

  • Completion of or on the Medicines Optimisation in Care Homes pathway, Clinical Pharmacists in General Practice education or Primary Care Pharmacy Education pathway
  • Postgraduate Diploma or working towards
  • A member of or working towards Faculty Membership of the Royal Pharmaceutical Society
  • Experience of providing clinical services to mental health or substance misuse patients
  • Experience of risk management
